To protect the club from liability, it is important that all participants fill out a waiver form before embarking on any club organized trips.   All current club members should have already filled out the forms.

Forms for new members and renewing members:

• MEMBERSHIP FORM FOR ADULTS - For new applicants and renewals.  Includes waiver form.
• MEMBERSHIP FORM FOR MINORS - For new applicants and renewals.  Includes waiver form.

Forms for non-members and course participants:

• General Waiver Form - For non-members taking part in a club trip.
• Waiver Form for Minors - For non-member minors taking part in a club trip.
• Waiver for Course Participants - For all participants in club instructional programs.
